Senior Care

Our providers offer primary care services in a variety of settings. Staff geriatricians, nurse practitioners, and pharmacists provide inpatient, outpatient, and nursing facility care. We treat patients with memory problems, incontinence, gait instability, falls, chronic illness, and complex drug regimens, as well as those who are in need of extended care.

Caregiver Support Group

Support for caregivers

Are you a caregiver of an elder or know of a caregiver who might benefit from education, resources, and the chance to connect with others experiencing similar challenges and emotions? We offer monthly caregiver support meetings for individuals caring for elderly loved ones and friends. Meetings take place at the Parkside Professional Building, Suite 440. Research and Education

Expanding the knowledge of our patients and families

The geriatric medicine staff is committed to expanding the fund of knowledge in clinical geriatric health care through ongoing clinical research programs. Senior Care physicians, nurse practitioners, and social workers serve as teachers to students and other physicians to promote knowledge, skills, and sensitivity to concerns of aging.

Extended Care

Extended Care is for patients residing in subacute care and nursing facilities served by our team. Hennepin Healthcare nurse practitioners and physicians visit patients and provide ongoing medical care as needed. We focus on patients needing rehabilitation after hospitalization and those needing long-term care. We can assist with the coordination of services when using our affiliated facilities.
