Community care at Hennepin Healthcare is provided by MVNA visiting nurses who see patients in the community wherever they call "home," whether it is a personal residence, care facility, or homeless shelter. Our programs provide care from prenatal and birth, to chronic disease management, to end of life.

Family Home Visiting

On January 1, 2023, Hennepin County will take on administration of the TeenHOPE, Pathways and Family Home Visiting programs.

Hospice

Exceptional end-of-life care focused on easing the physical, emotional, and spiritual pain that can accompany terminal illness.
