Family Home Visiting

Promoting family health, development and self-sufficiency

MVNA provides family-centered and community-based public health nursing services. Public Health Nurses (PHNs) work with families in their homes and other community settings. PHNs partner with health care providers and local community agencies to:

  • Promote healthy birth outcomes and positive parent-infant relationships.
  • Encourage healthy growth and development for infants and young children.
  • Prevent complications of illness and chronic disease conditions.
  • Promote healthy and safe environments.
  • Encourage family self-sufficiency, school attendance and use of community resources.

Note: We serve families in the City of Minneapolis and suburban Hennepin County with the exception of Bloomington, Richfield and Edina. If the client lives in Bloomington, Richfield or Edina, please refer to Bloomington Public Health at 952-563-8900, email or fax to 952-563-8997.


Home Visiting Services

  • Assessment of health status, home safety, depression, barriers to self sufficiency and use of community resources.
  • Health teaching and counseling related to pregnancy, birth, child growth and development, healthy lifestyle behaviors and personal safety. Multiple teaching methods are used including written information, audiovisual materials, role modeling and hands-on exercises.
  • Linking with community resources such as WIC, MFIP, schools, Early Childhood Family. Education, Follow Along, Child and Teen Checkups, Help Us Grow, Way to Grow and HUG.

Car seat classes

Clients are eligible to register for a car seat class and receive a free car seat if they are a member of one of the following health plans: BluePlus MA, UCare MA, or Hennepin Health. A car seat class help families learn which car seats to use and how to install them properly. Register early; class sizes are limited. Call to sign up for classes: 612-873-6963

Once you have registered by phone, a confirmation letter with your class date and location will be mailed. We bill your insurance for the cost of all classes.

TeenHOPE Program (Healthy Options for Parents through Education)

Case management and home visiting services provided  to teen parents up to age 21 receiving MFIP, HFA, or NFP benefits, who have not completed high school or received a GED.
