Home visiting services for teen parents

Services are eligible for teen parents who:

  • Live in Hennepin County
  • Are enrolled in a high school or GED program
  • Receive public assistance
  • Want to enroll in school or get a GED

Why home visiting?

  • Provide support as you work toward a high school diploma or GED
    • Public assistance requires school attendance
  • Parenting support and education
  • Health support and education
  • Family planning support and education

We also partner with HIRED to provide post-secondary planning and career exploration.  This collaboration is known as Pathways to Success.

Pathways to Success

We have two engagement sites for clients at Minneapolis Community and Technical College (MCTC) and at Brooklyn Center Academy (BCA) Link to Pathways.  The staff at each site help students decide which educational path is right for their individual situation.  This might include: catching up on and finishing high school coursework, preparing for the GED, or increasing Accuplacer college placement scores to enter a College Pathways program. Pathways staff work alongside public health nurses (PHN), social workers, or community health workers (CHW) to assist clients in reaching their educational goals.
