Family Connects is a nurse home visiting program for parents of babies born at Hennepin Healthcare’s Birth Center and who live in Hennepin County. A nurse visits you between 1-3 times after your baby turns 3 weeks old. You may have a visit earlier if your provider requests it.
Every family has needs when a child is born. The needs are different but we understand nobody is fully prepared for a baby. When we visit with you, our goals are to:
- Offer support and guidance on caring for your baby.
- Help you connect with your infant.
- Discuss community resources that can help you.
How do I get started?
When you have your baby at the Birth Center, you will be given information about Family Connects. A nurse will let you know that a Family Connects staff person will be contacting you shortly after you and baby leave the hospital. If at any time you decide you do not want a Family Connects visit, you can decline, either when we call you or you can call us.
