About the Clinic
The Hennepin Health Access clinic team helps patients to meet their social and psychological needs in addition to providing preventive health services and medical care for acute and chronic conditions. When patients have found better stability with support from the clinic, the team helps match them with providers for long-term primary care in more traditional primary care clinics.
This clinic is available by professional referral only. New patients to the clinic typically have frequent emergency department visits and need for multi-disciplinary support for medical, mental health, chemical health and/or social needs.
The Hennepin Health Access Clinic is co-located with the Coordinated Care Center and shares several resources and team members with that clinic team.
