“I enjoy working with patients and helping them understand their medications and how medications contribute to their health and health goals. I enjoy serving as a medication resource and building relationships with my patients and the providers in clinic. I want to help patients make a smooth transition from the hospital to home. My practice interests include cardiology and transitions of care.”
Clinical Expertise
Brooke received her Doctorate of Pharmacy degree at the University of Minnesota. She completed her residency training at the University of Minnesota Medical Center. She has been practicing medication therapy management at HCMC since 2013. Her role focuses on transitions of care, bridging patient care from the inpatient service to clinic.
