Hennepin Health Access

The Hennepin Health Access clinic provides team-based, transitional primary care to address the diverse needs of individuals referred to the clinic by providers within Hennepin Healthcare and/or the greater Hennepin County community.

About the Clinic

The Hennepin Health Access clinic team helps patients to meet their social and psychological needs in addition to providing preventive health services and medical care for acute and chronic conditions. When patients have found better stability with support from the clinic, the team helps match them with providers for long-term primary care in more traditional primary care clinics.

This clinic is available by professional referral only. New patients to the clinic typically have frequent emergency department visits and need for multi-disciplinary support for medical, mental health, chemical health and/or social needs.

The Hennepin Health Access Clinic is co-located with the Coordinated Care Center and shares several resources and team members with that clinic team.


For patients engaged with the clinic, the following services are available:

  • Chemical health services
  • Dental care
  • Diabetes Prevention Program
  • Food and clothing support
  • Housing and transportation support
  • Medication therapy management
  • Referral for employment services
  • Social services and case management


Hospital-based care

This is a hospital-based clinic owned by Hennepin Healthcare. Many insurance plans pay for health care services provided in a hospital-based clinic differently than those provided in a doctor’s office. Your insurance may require that you meet your annual deductible and/or require that you pay a percentage of the bill rather than just an office visit co-payment.
