Referral Process

Community Medical or Mental Health Provider

If you are a community medical or mental health provider and you would like to refer a young person to our clinic, please obtain the consent of the young person and their guardian, and call us at  612-873-6963.

Internal Hennepin Healthcare Provider

If you are an internal Hennepin Healthcare provider, please obtain the consent of the young person and their guardian and place a referral in Epic. To do so, select the "Referral to General Pediatrics and Specialties" option and choose "Gender and Sexual Health" within the order.

Reclaim
