MVNA is the community connections care division for Hennepin Healthcare. We provide home care in the community through safety net programs. Our programs provide care from prenatal and birth, to chronic disease management, to end of life and bereavement. We focus on prevention, hospital follow-up, transitional care, supportive services, public health, and community health initiatives.

When you donate non-medical health and safety items to Community of Caring, MVNA and hospice nurses can help clients to support their plan of care. With your help, Community of Caring is able to provide and meet the basic needs of clients during difficult times.
